The district nurse is available to coordinate with you to ensure that your child is healthy and ready to learn. The nurse travels from school to school but is available to each school site as needed.

The information you provide on the Student Health Information form will be used to alert us to any health conditions your child has that may require planning of care at school. This form should be completed annually for all students, usually done at time of enrollment.
Please keep your child’s school office or the district nurse informed of changes in your child’s health, medications, allergies, injuries, etc.
Please also alert your child’s school office to any changes in your contact information that occur during the school year. Keeping up to date information will help us to better serve you and your child.
Students with severe allergies, asthma, diabetes, seizures, or bleeding disorders will need additional information completed to keep them safe while at school. Click on Health Forms.
In case of Accident/Injury/Illness:
Each school has basic first aid supplies. Cuts and scrapes will be cleaned with soap/water. You will be notified if your child becomes ill and needs to go home.
If An Emergency Occurs:
It is necessary to inform your school’s office of phone numbers where parents can be reached if an emergency arises.
We also request names/phone numbers of 2 reliable friends/relatives who could assume temporary care of your child if an illness or an accident should occur and you cannot be reached.
If a parent/guardian or emergency contact person cannot be reached or is unavailable to pick up a student that needs medical attention, EMS may be called. Any expenses accrued will be the responsibility of the parent/guardian.
