NECESSARY PAPERWORK FOR ALL ATHLETES
 

1. Rank One: This is our system for all paperwork for every athlete at Guthrie High School & Guthrie Junior High School.

   2. To create a parent portal, you will need to provide your student's first and last name and date of birth.
2. On RankOne you will find the following forms to be signed electronically. Please do not turn in any paper forms EXCEPT the OSSAA Physical Examination and Parental Consent Form.

• Emergency Consent
• OSSAA Eligibility Form
• Biological Sex Affidavit
• Sudden Cardiac Awareness
• Concussion Acknowledgement
• Drug Testing Acknowledgement

All Physicals and Forms must be completed BEFORE any athlete can participate in practices or games.
